Use the Search page to find DPC practices by city, state, or ZIP code. You can also use the interactive Map to browse practices visually. Results can be filtered by specialty, services offered, and whether the practice is accepting new patients.

What does "accepting new patients" mean?

Practices marked as accepting new patients currently have availability for new memberships. If a practice is not accepting new patients, it may have a waitlist. Contact the practice directly to ask about availability.

How do I contact a practice I found on DPC 411?

Each practice listing includes contact information such as phone number, email, and website. Click on a practice from the search results to view its full profile with all contact details.

Is the information on DPC 411 accurate?

We work to keep all listings up to date and verify practice information regularly. However, details such as pricing, hours, and availability can change. We recommend contacting the practice directly to confirm current information before visiting.

Patient Accounts

How do I create a patient account?

Click "Patient Portal" in the header or footer, then select "Sign Up" to create a new account. You will need to provide your name, email address, and create a password. A verification code will be sent to your email to confirm your account.

What can I do with a patient account?

With a patient account you can save favorite practices, leave reviews, manage your profile information, and receive updates about practices you follow. Your saved information makes it easy to return and compare practices later.

I forgot my password. How do I reset it?

On the Patient Portal login page, click "Forgot Password" and enter your email address. You will receive a verification code to reset your password. If you do not receive the email, check your spam folder or contact us for assistance.

How do I delete my patient account?

To delete your account, log into the Patient Portal and go to your account settings. Select "Delete Account" and confirm. Alternatively, contact us at [email protected] and we will process your request. Account deletion is permanent and cannot be undone.

For DPC Practices

How do I list my practice on DPC 411?

Click "List Your Practice" in the header to begin the submission process. You will need to provide your practice name, address, contact information, services offered, and pricing details. Our team reviews all submissions before publishing.

How do I claim an existing listing?

If your practice already appears in our directory, you can claim it by visiting the practice page and clicking "Claim This Practice." You will need to verify your identity as the practice owner or authorized representative through our verification process.

How do I update my practice information?

Log into the Practice Portal with your credentials. From the dashboard, you can edit your practice profile including contact details, services, pricing, hours, and photos. Changes are published after a brief review.

Is there a cost to list my practice?

Basic listings on DPC 411 are free. We also offer premium listing options with enhanced visibility, featured placement, and additional profile features. Contact us for details about premium plans.

For Providers

How do I create a provider profile?

Click "Provider Portal" in the header or footer to access the provider registration. You will need to provide your professional credentials, specialties, and associate yourself with a DPC practice listed on our platform.

Can I be listed at multiple practices?

Yes. Providers who work at more than one DPC practice can associate their profile with multiple practice listings. Log into the Provider Portal and add additional practice associations from your dashboard.

How do I update my provider credentials?

Log into the Provider Portal and navigate to your profile settings. You can update your credentials, specialties, bio, and profile photo. Changes will be reflected on your provider profile after review.

Using the Website

Does DPC 411 work on mobile devices?

Yes. DPC 411 is fully responsive and works on smartphones, tablets, and desktop computers. The search, map, and all resource pages are optimized for mobile browsing.

How do I switch between light and dark mode?

Click the sun or moon icon in the header to toggle between light and dark mode. Your preference is saved automatically and will persist across visits.

I found an error on the website. How do I report it?
